FAO Patients transferred from Promenade Medical Centre - We would like to extend a warm welcome from the team at Palatine Group Practice (PGP) 

Unfortunately there is an inaccuracy in the letter you have received from DHSC specifically in relation to 'Patient Access'. We are unable to sign you up for this at the moment. Your previous access from Promenade unfortunately does not transfer across. Please order any repeat prescriptions in the interim by emailing and remember to include the pharmacy that you wish us to forward your prescription to for collection and as soon as we resume 'normal' services we will sign you up on your first visit to the Practice with Photographic ID and proof of address. New Patient Registration - Please note our list is currently closed and we are unable to accept new patients. If you are without a GP please contract Family Practitioner Services who will allocate you a GP practice. Thank you

PalatineIf you would like to join the patient list at Palatine Group practice, you can collect a registration pack from the surgery. (if there are additional members of your family, let the receptionists know who many packs you need bearing in mind there are different packs for adults and children).

When you have completed the forms, you should bring the back to the surgery, along with a valid (i.e. in date) photo ID and a proof of address.  As proof of address, we will accept a recent (within 3 months) utility bill, or a bank statement or similar.  We cannot accept a mobile phone bill.  Examples of photo ID would be a passport, a driving licence, or similar.  If your name has recently changed, e.g. if you have just got married, but you haven’t yet got your ID changed, please bring your marriage certificate with you.
