Have you registered yet for 'patient access'? This service allows you to book routine GP appointments, order repeat prescriptions, view parts of your medical record such as immunisation history, allergies and lab results. Please bring proof of identity to the practice to be given your unique online access code. This service is open to all patients aged 16 & over. 

New Patient Registration

PalatineIf you would like to join the patient list at Palatine Group practice, you can collect a registration pack from the surgery. (if there are additional members of your family, let the receptionists know who many packs you need bearing in mind there are different packs for adults and children).

When you have completed the forms, you should bring the back to the surgery, along with a valid (i.e. in date) photo ID and a proof of address.  As proof of address, we will accept a recent (within 3 months) utility bill, or a bank statement or similar.  We cannot accept a mobile phone bill.  Examples of photo ID would be a passport, a driving licence, or similar.  If your name has recently changed, e.g. if you have just got married, but you haven’t yet got your ID changed, please bring your marriage certificate with you.
