FLU VACCINES. We now have our vaccine in stock and we are booking appointments for all patients over 65 and those who are in an 'At risk' category. Our clinics will be limited to Saturdays in October, this is to avoid impacting the Nurses already very heavy workload on weekdays. Please call on 623931 to secure your appointment. Kind regards, Ali, Practice Manager

New Patient Registration - Please note our list is currently closed and we are unable to accept new patients. If you are without a GP please contract Family Practitioner Services who will allocate you a GP practice. Thank you

PalatineIf you would like to join the patient list at Palatine Group practice, you can collect a registration pack from the surgery. (if there are additional members of your family, let the receptionists know who many packs you need bearing in mind there are different packs for adults and children).

When you have completed the forms, you should bring the back to the surgery, along with a valid (i.e. in date) photo ID and a proof of address.  As proof of address, we will accept a recent (within 3 months) utility bill, or a bank statement or similar.  We cannot accept a mobile phone bill.  Examples of photo ID would be a passport, a driving licence, or similar.  If your name has recently changed, e.g. if you have just got married, but you haven’t yet got your ID changed, please bring your marriage certificate with you.
